Becoming a HealthCare International Appointed Intermediary is simplicity itself. Complete a copy of our Appointed Intermediary Application Form (this can be downloaded by clicking on the link below) and then return it to us by fax at +44 (0)20 7590 8815.

Once your application has been approved, we will send you your unique intermediary code along with a useful folder containing copies of all our literature and documentation.
