your application form now completed, simply print off a copy and send
to us either by fax on +44 (0)20 7590 8815 or by post to: HealthCare International Global Networks Ltd UK Administration Centre, 160 Brompton Road, Knightsbridge London SW3 1HW United Kingdom Once we have received your completed form and payment details, your application will be processed and we will normally be able to confirm that your cover is in place within 72 hours. Please remember that even if you fax the application form to us we will still require the original in due course. So please mail the original at the same time. |
